1. Hospitals/Nursing Homes
- Duration: 2 years (April 1, 2025, to March 31, 2027), extendable by 1 year.
- Eligibility:
- Registration/Permission from the State or District Health Authority.
- Details of facilities such as the number of beds, specialities, diagnostic labs, emergency services, and blood banks.
- NABH accreditation preferred.
- Rates offered to ONGC with a minimum validity of 3 years.
- Documents Required:
- Proof of registration.
- NABH certificate (if available).
- List of facilities and rates charged from the general public.
2. Diagnostic Centers (Pathology/Radiology)
- Duration: 3 years (April 1, 2025, to March 31, 2028).
- Eligibility:
- Registration/Permission from the State or District Health Authority.
- NABL accreditation preferred.
- Attached Pathologists/Radiologists must have MD with a minimum of 3 years of experience.
- Rates offered to ONGC with a minimum validity of 3 years.
- Documents Required:
- Proof of registration.
- NABL accreditation certificate (if available).
- Details of attached doctors and facilities.
3. Consultants (Specialists/Super-specialists, Dentists, Physiotherapists)
- Duration: 3 years (April 1, 2025, to March 31, 2028).

- Specialists: MD/MS with 5 years of experience.
- Super-specialists: M.Ch/DM with 3 years of experience.
- Dentists: MDS with 3 years of experience.
- Physiotherapists: B.Sc. in Physiotherapy/Occupational Therapy with 3 years of experience.

- Certificates of qualification and experience.
- Valid registration from Medical/Dental/Physiotherapy councils.
- List of facilities and rates charged from the general public.
